DAVENPORT, FL — AdventHealth Heart of Florida and AdventHealth Lake Wales have opened the application period for the 2026 Community Impact Grant. This initiative provides one-year grant funding to local nonprofit and community-based organizations to expand community health programs and strengthen access to whole-person care throughout Polk County.
The grant aims to address the area’s most pressing health needs by supporting projects that deliver measurable improvements in community well-being. Funding is available in various amounts for organizations serving residents across Polk County.
Priorities for the 2026 grant cycle are based on the 2025 Community Health Needs Assessments (CHNA) for AdventHealth Heart of Florida and AdventHealth Lake Wales. These assessments identified mental health, health care access and quality, and nutrition and healthy eating as the most significant needs in the region.
The program focuses on evidence-based, programmatic initiatives that enhance quality of life, reduce barriers to care, and promote healthier communities. Eligible organizations must demonstrate the ability to deliver meaningful and sustainable outcomes, collaborate with community partners, and track progress through measurable metrics.
Eligibility requirements include:
- Providing programmatic services in Polk County aligned with mental health, health care access and quality, or nutrition and healthy eating, with priority given to organizations serving communities around AdventHealth Heart of Florida and AdventHealth Lake Wales.
- Demonstrating sustainability of both the organization and the proposed program, including plans to maintain the initiative beyond the grant period.
- Providing quarterly data to AdventHealth to track program impact and progress.
- Identifying necessary partnerships or collaborations to support program success.
- Serving residents living within Polk County.
In 2025, AdventHealth Heart of Florida and AdventHealth Lake Wales invested more than $375,000 in nonprofit organizations and community partners across Polk County. This funding strengthened essential programs that improve health, housing, behavioral wellness, and public safety, while expanding access for more local residents.

“Strengthening the health of our community requires more than exceptional clinical care, it requires strong partnerships with the organizations already doing transformative work in neighborhoods across Polk County,” said Tim Clark, AdventHealth Polk Market President and CEO. “This grant program allows us to join with mission-driven leaders whose programs uplift families, open doors to support and create lasting opportunities for whole-person well-being. Together, we can build a future where every person has access to the critical resources they need to feel whole.”
Applications must be submitted by June 1, 2026. No late submissions will be accepted. Community organizations across Polk County are encouraged to apply.
